What is your policy on insurance, medicare and Medicaid?

OPAL Medical Clinic does not contract directly with Insurance companies, Medicare or Medicaid at this time. Hence, we are considered "out of network". Portions of your bill (e.g.50-70%) may be reimbursed by your insurance, depending on the type of insurance you carry i.e. POS or PPO with out of network priveleges. We cannot guarantee reimbursement but will provide a receipt with ICD and CPT codes to assist with the claim. Please check with your insurance provider. We are not enrolled with Medicare or Medicaid. We do accept cash, major credit cards and checks.

We do however offer a Membership program, which would be ideal for someone who wants to be under our care for the whole year, and believes in our philosophy of optimal aging & wellness. You will have access to our doctor for all scheduled visits during the year under a retainer scheme, instead of paying per visit. Also, please note that Health Savings Account (HSA) may be available through your employer if you have a high deductible insurance. This is different from Health Reimbursement Account (HRA) or FLEX. HSA are tax free and can be rolled over to the future and is portable should you change jobs. We do highly recommend HSA for our patients, as getting reimbursed for fees is much simpler and quicker. You will also have a better choice of doctors and may work out better for preventive health programs such as ours.

We use the AMAapproved CPT codes for our fees. Consultation rates or professional fees are time and complexity based.

1. The first consult, which typicaly last an hour costs approximately $300-400
2. Follow up charges are $150-200 usually
3. Blood work is extra and run about $100-300 depending on tests ordered
4. If indicated, treadmill stress ekg with physician interpretation runs about $500 plus, whereas a regular ekg with interpretation cost about $100
5. Complete executive physicals will depend on the tests indicated and will be discussed prior to ordering any of the tests. Some of it might be covered by insurance.
Payment is expected on day of service. You will get an itemized breakdown for all fees on checking out. This could by supplied to your insurer or your health savings account (HSA) for claims. You can verify prices before the consultation and a breakdown list is provided on request.

We also offer a Membership Program, which is s a retainer for the doctor's professional services for 12 consecutive months, to include initial and follow up visits as well as an annual complete physical and stress EKG test.

Our chronological age cannot change, but we can alter our biological or physiological age. It is indeed possible if you work at it. Often times, it may be as “simple” as weight loss, exercise and diet. Our programs may not be for everyone, and we do have to turn down clients sometimes.

Yes, through partner-compounding pharmacies that we refer to.

We will give you copies of your laboratory tests etc.

We will refer you to the appropriate specialist, or if appropriate we will commence treatment ourselves.

Yes. We see women for wellness and general physicals. We also do in depth analysis of hormonal problems e.g. menopause and female androgen insufficiency syndrome. However, we do not do PAP smears and other gynaecological procedures; but we will be happy to refer you to the appropriate specialists for these procedures.

For the convenience of our patients, telephone consults are available for established patients at the OPAL Medical Clinic for follow up purposes. Payment will be based on the time spent discussing the issue & will be at the same rate as established office visits. Patient will be invoiced after the call. There is no charge for new patients enquiring about care & services at the OPAL Medical Clinic.

At this time, Dr Tan is usually at this clinic on Tuesdays and Thursdays mornings and Wednesday afternoons.
You should expect a return email or call back to confirm the time of the appointment within 24 hours, unless we are closed or is a holiday. Please do not turn up for the appointment till we confirm it. Also, if you do wish to cancel your appointment, please let us know within 24 hours or there will be a charge.
